It does hurt, but no more than anything else that's been happening to him. For Dean, everything seems to go white, until he hides his face against Cas, and then the next thing he knows is that he's outside the gas station. Hell is so far away, and Cas is here, right here. Dean's still holding onto him, and his smile is tired but bright.
